FinalForms allows you to review and electronically sign athletic participation forms for your students. The best part is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time.<\/span><\/p>\n<p><span style=\"font-weight: 400\">You may review your data at any time to verify it is current. If you require any support during the process, scroll to the page bottom and click \u201cGet Help\u201d.<\/span><\/p>\n<p><span style=\"font-weight: 400\">We are asking the parents\/guardians of all students planning to play sports to create accounts in FinalForms and register your student(s).\u00a0 <\/span><b>A parent must create a PARENT account to register a student for sports.<\/b> <span style=\"font-weight: 400\">Students can not register themselves through a student account.<\/span><\/p>\n<p><span style=\"font-weight: 400\">When you register, <\/span><b>make sure that for all required forms, the electronic signature for both the Parent and the Student are completed<\/b><span style=\"font-weight: 400\">. Additionally, <\/span><b>it is <\/b><b>required<\/b><b> that you upload a copy of the student&#8217;s current physical to the document section of the registration.<\/b><span style=\"font-weight: 400\"> This allows our nurse to view and approve each student athlete. style=\"font-weight: 400\">Dear Student Athletes and Families,<\/span><\/p>\n<p><span style=\"font-weight: 400\">Greetings to our returning NMHS Student-Athletes and a big welcome to our incoming Freshman Class of 2028!\u00a0 Although Summer vacation is just about to start, we are already planning for the upcoming 2024 Fall Season which will be here before we know it.\u00a0 This letter will provide you with some important information you will need for the 2024 Fall Sports Season.\u00a0 We will be having our Fall Sports Information and Meet the Coaches Night on Monday, June 10<\/span><span style=\"font-weight: 400\">th<\/span><span style=\"font-weight: 400\"> at 5:00PM in the NMHS Theater.\u00a0 You will be provided information regarding Athletic Department Policies and Expectations, Medical Information, Online Registration<\/span><span style=\"font-weight: 400\">\u200b<\/span><span style=\"font-weight: 400\">, Academic Eligibility, CIAC Information, and other important topics.\u00a0 All prospective Fall student-athletes are expected to attend this meeting with a parent\/guardian.\u00a0 After my 30 minute presentation, teams will separate into breakout rooms to meet the coaches of the sport you are interested in.\u00a0 This individual sport meeting will provide the opportunity to meet the coaching staff of your child&#8217;s potential sport, set up team communications, get specific information about the tryout schedule, philosophy, and team rules\/expectations. Please check your physicals to make sure they are current. It is necessary that a SIGNED, UP TO DATE PHYSICAL IS UPLOADED TO THE REGISTRATION PAGE once registration is open.\u00a0 The ENTIRE physical must be uploaded.\u00a0 Physicals are good for 13 months.\u00a0 Again, instructions and links will be communicated when registration is up and running.\u00a0 Without completing the registration and uploading an up to date physical, a student <\/span><span style=\"font-weight: 400\">may not<\/span><span style=\"font-weight: 400\"> participate in any practices or games.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">Our coaches will be provided with a report of students on the first day of practice which will tell them who is approved or unapproved.\u00a0 Please make sure to register ahead of time to avoid any delays.\u00a0 Additionally, please review the Athletic Handbook on the high school web page so you are familiar with the expectations and policies of the New Milford High School Athletic Department.\u00a0 Our Coaches will also set up their own format of communication of team information.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">A reminder that our Fall Sports include:<\/span><\/p>\n<p><span style=\"font-weight: 400\">\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0\u00a0&#8211;
