School Based Health Center

New Milford Public Schools and CT Institute for Communities (CIFC Health) working collaboratively together to support student’s and family’s needs within the district providing mental health services year round.

What is a School Based Health Center?

The purpose of the School Based Health Center (SBHC) at this time is to provide mental health/behavioral support to students, families, and community year round. Our goal is to work collaboratively with school staff to provide therapeutic support for students during the school day to prevent students from being removed from school for therapy sessions, decrease crisis at school/home, and help reduce families who struggle to utilize therapeutic services within the community.  Services are provided by a licensed mental health provider located within the school building.

How to enroll?

A student MUST be registered to receive services. Registration forms can be found at the school based health center, link below, or can be sent home via email. Sign up even if you do not use services the registration form will follow you throughout your schooling in NM and if you choose to attend Henry Abbott Technical High School, we have a SBHC there too.

The SBHC accepts students with or without insurance. If you do have health insurance, we do require a copy of the insurance card to be sent in with the student’s registration form. If you do not have insurance your SBHC will connect families with the CFIC Health Eligibility Department to help gain access to insurance or set up a sliding scale fee.  The SBHC works like a normal outpatient clinic inside your child’s school. CIFC Health will bill insurance and a monthly invoice will be sent to you regarding the balance you are responsible for.

Student Registration

* Online Registration Is Preffered

When registering a student, you must provide:

  1. The child’s birth certificate. Age of Entrance (See policy 5111)
  2. Proof of residency or homelessness (See Administrative Regulations Regarding Nonresident Students 5175 R)

 

All children living in the in the US have the right to a free K-12 public education. Children do not need a green card, visa, passport, alien registration number, or social security number to register for school.

Looking to Volunteer
or Chaperone?

The Board of Education recognizes that volunteers can make many valuable contributions to our schools. The Board endorses a volunteer program that encourages community residents to take an active role to become school volunteers subject to suitable regulations and safeguards.

The New Milford Public Schools recognizes that just about any volunteer situation may result in direct contact with students.  Our obligation to take appropriate measures to keep our children safe is one of our primary concerns. We will be requiring criminal history checks, at no cost to you, of all adults who wish to act as volunteers in our schools or chaperones on our field trips.
